Alas, I wish I could say I’ve never seen that….but my beloved wife came to me one day complaining her pistol “was broken” found that exact situation inside it
As ejection is typically to the right, that round is [nearly] chambered backwards. Yikes.
A couple hits on the back of the slide should fix it. ?

It is called a Phase 3 malfunction. They will ruin you day in a match, and kill you in a gunfight.
Yum, that bullet has rifling marks on it. I think that what we are seeing is a round that entered via the muzzle and stopped when it jammed at the chamber in.
I’m not sure that is a round in the mag backwards issue.
An old-school shooter wasn’t up to date on breech-loading ordnance?
I’m not sure what those marks are. First guess was that somebody had already gone after it with pliers, but the crimp marks on the case look awfully regular, in a highly irregular way.
Looks like someone has been working at it. Some look like vise grip marks.
And apparently didn’t come up with the obvious removal technique: drop a large nail down the barrel and give it a good whack with a hammer.
(I suppose if this sort of malfunction were more common, we’d see caliber-specific hollow-tip rods just for the purpose.)
A large nail, to hit the stuck cartridge right in the primer? Hm.
That WILL get it out of the barrel…
That’s why I picked a nail instead of a rod. Because I’m “helpful” that way.
I have seen this malfunction twice in the last ten years. Once at 3gun and at IDPA. They swear the round wasn’t loaded backwards. I don’t know, but it’s a marvelous way to fail badly on a stage.
