Looking at the right hand slide lock, maybe a Grand Power?
Doesn’t look catastrophic to me. It flew apart as it was cycling. The hammer is back and the parts appear to be intact. Looks like an ID-10-T error. Or the takedown lever broke allowing the spring to simply yeet the slide forward.

What would do this? Excessive lightening resulting in some critical part failing because it was cut down too much?
